Preparing For Your visit

Thank you for booking a visit with me! Here’s a checklist of items to review to help you get ready for your visit.

Prior to your appointment

You should have received an email to complete intake and consent forms. It is crucial to fill these out prior to the appointment so I can have a clear picture of what is happening with the parent and/or baby. Filling these out during the appointment will cut into our appointment time, and I would rather spend that time working with you on what is going on! If you have not received a link to access your forms, please contact me. 

If there is anything important I should know about directions or parking at your home, please let me know prior to our visit.

Feeding before our appointment

Please try not to feed your baby within 90 minutes of our appointment so we can observe a good feeding together and get an accurate pre and post feeding transfer weight. If your baby is unable to wait until our visit, you can offer a short feeding or small amount in the bottle if needed. If you feel like this may be the case, please contact me and I can see if I can arrive earlier!

Pets

I love them, but please kindly have them in a separate room for the duration of our consultation. Some pets can be unpredictable when a new baby arrives, especially around strangers, and I want to ensure everyone stays safe. I’d love to keep my focus and you and our discussion.

Equipment

If you are using any tools to aid you in the feeding process, i.e. breast pump, nipple shields, nursing pillows, at-breast supplementer, formula, expressed milk, syringes, cups, and bottles, etc., please have these available during our visit so can review their usage as needed.

During the consult

  • I will text you if I am running behind or ahead of schedule as needed close to the start time of our appointment. 
  • I will wash my hands as often as needed, please have liquid soap (dish soap ok) and paper towels available in your kitchen. 
  • Please clear a space on your table or floor for my scale, this requires a stable surface. 
  • I will be asking for your WiFi password, if it is possible for you to share!

Cancellation Policy

SELF PAY VISITS:

You must notify the lactation consultant at least 12 hours prior to the appointment to either be granted a full refund or rescheduled to a different time. If you do not notify the lactation consultant of canceling the appointment at least 12 hours prior to the appointment, or you do not show up to the appointment, no refund will be given and you will need to completely reschedule your appointment, paying in full for another appointment. 

If you do not arrive or communicate with Melissa Cheng at Lucky Lactation within 15 minutes of the scheduled appointment start time, your appointment will be considered a "no show"; no refund will be given and you will need to completely reschedule my appointment, paying in full for another appointment. If you call or arrive within 15 minutes of the original appointment start time, you may keep the current scheduled appointment, but no additional time will be credited and the original end time will remain as scheduled. 

INSURANCE COVERED VISITS:

You are responsible for all charges associated with this visit. If you cancel within 48 hours' notice, you will be billed $100 and 25% of the travel fee (if applicable).

A 100% travel fee will be charged for a no-show for a home visit.

COVID / Illness Policy

SELF PAY VISITS:

You must notify your lactation consultant of any illness within your household before or immediately after our in-person appointment. Your lactation consultant reserves the right to cancel your appointment and you are expected to pay in full for the appointment and reschedule a new appointment:

  • If your lactation consultant arrives to our scheduled appointment, and anyone in your household is actively sick with symptoms including coughing, sneezing, fever
  • If anyone in your household, or has had regular contact with you or your baby, has tested positive for COVID within 48 hours of my scheduled appointment 

If you notify your lactation consultant in an appropriate amount of time, you may reschedule your appointment per the guidelines listed above without penalty or fee. 

INSURANCE COVERED VISITS:

If an in-person visit is canceled in less than 48 hours due to COVID-19 exposure,

COVID-19 symptoms, or other contagious illness, then there is no cancellation charge for rescheduling to telehealth at the same appointment date and time. If the appointment is cancelled and not rescheduled, there is a $100 cancellation charge that is not reimbursable by insurance.
